3D visualization of myocardial substrate using Delayed Enhancement MRI for pre-planning and guidance of ablation procedures of ventricular tachycardia

چکیده
Background In patients with prior myocardial infarction, intra-scar surviving fibers may create conducting channels (CC) which are the substrate of most sustained monomorphic ventricular tachycardias (VT). These channels can be identified by endocardial voltage mapping. Recent studies show high correlation between information from endocardial voltage mapping and delayed-enhanced MRI (DE-MRI) images.

Integration of merged delayed-enhanced magnetic resonance imaging and multidetector computed tomography for the guidance of ventricular tachycardia ablation: a pilot study.
BACKGROUND Delayed enhancement (DE) MRI can assess the fibrotic substrate of scar-related VT. MDCT has the advantage of inframillimetric spatial resolution and better 3D reconstructions. We sought to evaluate the feasibility and usefulness of integrating merged MDCT/MRI data in 3D-mapping systems for structure-function assessment and multimodal guidance of VT mapping and ablation. METHODS Nin...
متن کاملMRI-Guided ventricular tachycardia ablation: integration of late gadolinium-enhanced 3D scar in patients with implantable cardioverter-defibrillators.
BACKGROUND Substrate-guided ablation of ventricular tachycardia (VT) in patients with implanted cardioverter-defibrillators (ICDs) relies on voltage mapping to define the scar and border zone. An integrated 3D scar reconstruction from late gadolinium enhancement (LGE) MRI could facilitate VT ablations. METHODS AND RESULTS Twenty-two patients with ICD underwent contrast-enhanced cardiac MRI wi...
متن کاملTowards cardiac and respiratory motion characterization from electrophysiology data for improved real time MR-integration
Background Electro-anatomical voltage mapping (EAM) is an invasive technique used for the identification of ventricular tachycardia (VT) substrate and subsequent guidance of VT ablation [1]. The mapping of VT substrate is very timeconsuming procedure, requires highly skilled electrophysiologist, is associated with patient risk and is an invasive procedure. Late gadolinium enhancement (LGE) MRI ...
متن کاملFeasibility of real time integration of high-resolution scar images with invasive electrograms in electro-anatomical mapping system in patients undergoing ventricular tachycardia ablation
Background Ventricular tachycardia (VT) ablation is generally guided by invasive mapping of the left ventricle (LV) using electro-anatomical voltage mapping (EAM) to identify the VT substrate [1]. Late gadolinium enhancement (LGE) MRI allows excellent visualization of the scar. Heterogeneous area in LGE images has been shown to correlate with the VT substrate in animal models of VT. Retrospecti...
متن کاملMagnetic resonance-based anatomical analysis of scar-related ventricular tachycardia: implications for catheter ablation.
In catheter ablation of scar-related monomorphic ventricular tachycardia (VT), substrate voltage mapping is used to electrically define the scar during sinus rhythm. However, the electrically defined scar may not accurately reflect the anatomical scar. Magnetic resonance-based visualization of the scar may elucidate the 3D anatomical correlation between the fine structural details of the scar a...
